Howto install kernel or using *.patch?

Hi..
I have a distro with older kerrnel version.
I have download kernel source from kernel.org
(~45megs).
I study the HOWTO, but it makes me confused.
How to instal the new kernel or how to just use patch
file (*.patch) to patching the existing kernel to a
new one?
