Editing PDF's in Linux

While it's technically possible to search/replace PDF text like you're
asking (the full Windows version of Acrobat allows you to freely
edit the text--but only within a single line, meaning multiline
comment boxes would not be easy), the web-based form idea mentioned
by Jonathan is your most flexible option.

Does the PDF page you're printing have a complex layout? If it
is pretty simple, then code it in PHP and have Mozilla print it.
Even with graphics, Mozilla renders HTML quite nicely to the printer.

Or use something like Open Office to do so without any coding.
