How I would do it:

Audacity, the OSS audio editor. Record the entire disk to a single
Audacity file. Once recorded, visual walk through the file finding the
silence points to break the tracks at. On each track, click the break
point. Select from point to beginning of track using the Edit menu.
Right click the track and click "split". After all tracks are split,
select each track and "Export Selection" to WAV. Once all of the .wav's
are exported, you can convert them all to (.flac|.ogg|.mp3) with a
single command line like 'ogg123 -q5 *.wav'.

Using this method, you can also remove needle hiss with the filters in
Audacity.
