I second the PVR-250, it's what I use in my MythTV box at home and I
haven't had any complaints.

I would definitely go with a Hauppauge PVR 250. It is very well tested
within myth. I have a myth box with one of these cards and it was a
snap to set up (FC3 following Jared Wilson's guide). It also has the
remote that is well documented and performs nicely. I am looking to add
another card to the setup so I can watch TV and record at the same
time.
